We are excited to welcome you to our Father’s Day Fiesta tournament!

NEW! While we expect coaches to collect and have their signed medical release forms at all tournament games for all rostered players, we are not collecting these forms as a team check in document. Instead, coaches will certify that they have these documents on the team check in form.

Roster Entry Deadline – Saturday, June 7

Please log in to your Playmetrics account and follow these steps to enter your team’s roster:

 Go to “My Teams” (do not click the purple "Register" button—your team is already registered)
 Click on your team name
 Click on “Manage Roster”
 Enter the following for each player and staff member:
o Player name
o Player date of birth
o Player jersey number
o Contact info for coaches, assistant coaches and/or team managers

Once all players information is inputted, click on “More Actions” tab to edit jersey numbers.

Tournament Check-In – Saturday, June 20

Before your first game, you must check in at your game site’s headquarters and turn in these items for team check in:

​

1. Official team roster signed by club administrator. You may use our official roster form or one generated from your club with a verifying club official signature. Each player  must have a unique roster number. Tape or marker may be used to create a unique number.

2. Team Check-In Form

Coaches will be required to certify on the team check in form  that they are in possession of completed medical release forms signed by the parent and/or guardian and that the forms will be in their possession at every game.  The medical release form in possession by the coach can
be a tournament medical release forms, club release forms or other generic release forms.
